Community Engagement Process for CCHOHT

Challenge

Collective Good Consulting was engaged to conduct a comprehensive engagement process involving patients, families, caregivers, primary healthcare providers, and partners in the Connected Care Halton Ontario Health Team (CCHOHT) geography of Oakville, Milton and Halton Hills. This initiative aimed to identify and document the needs and requirements for digital enablers in the CCHOHT. Digital enablers are applications or technologies designed to facilitate communication and workflow processes, enhance user experience, provide value, improve health outcomes, and enrich the clinical experience.
